During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site renewal inspection was conducted on 11/15/24. The on-site inspection began at 11:17am and ended at 12:48pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 15. The inspector reviewed 5 children?s records, 1 caregiver record and 1household member record while on-site.
This inspection included document review, tour of the facility and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Adult household members shall have a completed sworn disclosure statement (SDS).

Household member #2 did not have a completed SDS on-file.
Plan of Correction: The provider said a SDS will be completed and kept on-file.
Standard 8VAC20-800-170-A
Each adult household member shall have a current TB screening.

Household member #2 did not have documentation of a completed TB screening/test.
Plan of Correction: The provider said a TB test/screening will be done.

Repeat violation:
A child's record shall contain the name and policy number of the child's medical insurance.

The record for Child #7 listed an acronym for the insurance name but no policy number.
Plan of Correction: The provider said she will have parents provide the information.
