Inspector Notes

An unannounced 60 day monitoring inspection was conducted on 8/23/2023 from 12:52 pm-1:30 pm. There were 9 children supervised by 3 direct caregivers. The home, 3 staff records, 3 children?s records, evacuation drills, and policies were inspected. There were no medications on site today. Children were observed napping. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on record review and interview, the provider did not obtain all the information required for a child's record.

Evidence: Child #1 is missing the addresses of two designated persons to contact in case of an emergency if the parent cannot be reached.
Plan of Correction: Information required was requested to the parents.

Based on record review and interview, proof of identity was not obtained within 7 days.

Evidence:
1. Child #1 does not have signed documentation of viewing the proof of identity for the child.
2. The start date for child #1 is 7/11/2023.
Plan of Correction: Proof of identity was requested immediately.
