Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

A Subsidy Health and Safety Inspection (SHSI) was conducted on 7/3/2025. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/22/25. A POC submitted after this date will not appear on the public website.

The vendor shall obtain documentation of additional immunizations once every six months for children less than the age of two years. The most recent immunization record for child #1 was dated more than 10 months ago. An updated immunization record was 4 months overdue.
Plan of Correction: The provider responded: The parent provided the up to date immunization document.
Standard 8VAC20-790-280-A
Repeat Violation
Each bathroom area provided for children shall have running water that does not exceed 120?F. The water temperature at the bathroom sink measured 131?F when measured with a thermometer.
Plan of Correction: The provider responded: The water temperature has been turned down.
Standard 8VAC20-790-390-A
Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. Child #1 and child #2 sat down at the table and began to eat lunch without washing their hands.
Plan of Correction: The provider instructed the children to go wash their hands.
