Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 26, 2025, as a part of the licensure period. The on-site inspection began at 10:10am and ended at 11:10am. The inspector reviewed compliance in the areas listed above. There were four children present and three caregivers; the point total was 14. The inspector reviewed five children?s records and three caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-120-B
Assistants and substitute providers' records shall contain documentation of two or more references as to character and reputation as well as competency and checked before employment.

Caregiver #2, employed for four days, did not have documentation of a second reference.
Plan of Correction: The provider conducted a reference check by telephone the day of the inspection. Moving forward, the provider will ensure to include documentation of two references in the record.
Standard 8VAC20-800-960-E
If more than one infant is in care, bottles shall be refrigerated and labeled with the child's full name and date.

Two infants are in care. Four of six bottles were not labeled with the child's full name and date.
Plan of Correction: The bottles were labeled with the children's names and date at the time of the inspection. Moving forward, caregivers will ensure to write the child's full name and date on the bottles upon arrival.
