Inspector Notes
A monitoring inspection was conducted on 07/18/2023. The inspector was on site from approximately 9:00 am - 10:25 am. There were a total of 5 children (17 points) observed with 2 caregivers. The provider was not on site during the inspection. The children were engaged with the caregivers. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 5 child records and 2 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Caregiver 1 and 2 (DOH 7/17/23) did not have the results of a nationwide fingerprint search on file.
Nationwide fingerprint search was processed for caregiver 1 on July 22nd, 2023.

Caregiver 1 and 2 did not have a sworn disclosure statement on file during the inspection.

Caregiver #1 was missing verification of age, job title, date of employment, two references, background checks as required by 8VAC20-770, documentation of TB screening as required by 8VAC20-800-170, and documentation of education and training as required by 8VAC20-800-230.
Caregiver #2 was missing name, address, verification of age, job title, date of employment, two references, background checks as required by 8VAC20-770, documentation of TB screening as required by 8VAC20-800-170, documentation of education and training as required by 8VAC20-800-230, and documentation for name and telephone of a person to call in case of an emergency.

The provider left the family day home operations under the guidance of 2 caregivers for approximately 5 hours that were not qualified to be substitutes and that did not have the required paperwork to be a caregiver.
Evidence: There was no documentation of notifying parents that the provider would not be in the family day home an extended amount of time.
