During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 04/03/2025. The on-site inspection began at 10:30 am and ended at 11:35 am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers; the point total was 25. The inspector reviewed 3 children?s records and 3 caregiver records on-site on 04/03/2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/16/2025. A POC submitted after this date will not appear on the public website.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

Provider arrived within 30 min. of initiated inspection due to licensing required medical exam being completed.
Standard 8VAC20-800-920
Menus. When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated.

The menu that was posted was not dated for the current one week period.
Plan of Correction: Menus are dated daily for parents via app, however we have made the correction to date our internal menu manually as well.
Standard 8VAC20-800-960-E
Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.

Two sets of bottles were not dated.
Plan of Correction: Bottles received from parents are always refrigerated unless we have utilized it, which then will require for them to be outside. Parents were reminded to include dates on bottles daily.
