Inspector Notes

An unannounced monitoring inspection was initiated on 7/31/2024 and completed on 7/31/2024 in response to a self-report received by the licensing office on 7/23/2024 relating to supervision and administration. This inspection included document review of one accident report, policies and procedures, memos, emails, interviews and observation of video footage recording.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 12, 2024. A POC submitted after this date will not appear on the public website

Standard 8VAC20-780-340-A
Repeat Violation
Based on observation of video footage recording and interviews, staff failed to always ensure the care, protection and guidance when supervising children.
Evidence:
1.On July 22, 2024, at approximately 4:10pm, two children (ages 4 and 5 years) were observed physically fighting over a soccer ball and wrestling on the ground of the soccer field for approximately 1 minute and 20 seconds. The three staff on the playground and responsible for supervising the group of 20 children were unaware of the incident until the parent of one of the children arrived on the playground and intervened by yelling.
2. One staff was sitting and talking with 3 children, one staff was talking with a parent with her back to the children, and the third staff was facing the children but did not notice the fighting until she heard the parent yell.
Plan of Correction: Rigorous re-training of appropriate supervision will be done with ALL staff
members. Understanding the signs of inappropriate behaviors and interactions amongst all children will also be a topic of re-training for ALL staff. Review of our Employee Handbook will be required and each staff will sign off that they reviewed it.
Standard 8VAC20-780-40-E
Based on documentation review, observation of video footage recording and interviews, the center did not ensure that the center's activities and services were maintained in compliance with these standards and the center's own policies and procedures that are required by these standards.
Evidence:
1. The center's policy on playground and supervision states: "Team members should be involved with children, observing them at play and guiding them toward productive activities. Ninety (90) percent of all accidents in preschools happen on playgrounds, so active supervision of children is a must. Please do not congregate in groups with other teachers and chat. Observe the "triangle rule" and maintain stations amongst the children to ensure their safety . Chairs are not allowed outside. In order to maintain maximum supervision, sitting is not allowed.?

2. On 7/22/2024 at approximately 4:10pm two children (ages 4 and 5 years) were observed physically fighting and wrestling on the ground of the soccer field for approximately 1 minute and 20 seconds. 3. Staff #1 was observed sitting on the playground speaking with three children. She stated she did not see the incident.
4. Staff #2 was observed with her back to the children while speaking with a parent for approximately 5 minutes. She stated that she also did not see the incident.
5. Staff #3 was observed looking at Staff #2 speaking to the parent, then looking to the right, speaking with a child and then looking in the middle area of the playground. Staff #3 was standing directly across from where the children were fighting/wrestling and did not intervene to stop the children from fighting. She stated she did not see the incident until she suddenly heard the parent state ?Oh no, don?t hit my son.?
6.The three staff responsible for the 20 children were not observing the triangle rule and maintaining the stations amongst the children. All three staff were spread out in one line. Staff #3 should have been positioned across the field in the middle near where the incident occurred..

interactions amongst all children will also be a topic of re-training for ALL staff.
Review of our Employee Handbook will be required and each staff will sign off that
they reviewed it.. A demonstration will be done as well as frequent walk-throughs
by Leadership to ensure that the 'triangle' positions are maintained.
