During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated and completed on 2/04/2025 in response to a self-report received by the licensing office on 1/31/2025 relating to behavioral guidance and forbidden actions. The inspector reviewed one staff record on-site on 2/04/2025. This inspection included document review, interviews and observations.

The information gathered during the inspection of the self reported incident determined no violations with applicable standards or law.

Though not a part of the original self report, violations that were not reported were found related to background checks and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/12/2025. A POC submitted after this date will not appear on the public website.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years prior to employment.

Staff #1, employed for 11 months, did not have documentation of the Out-of-State Criminal History Records Request or Out of State Sex Offender Registry Search from one state prior to employment.The center obtained Out of State Sex Offender Registry results during the inspection and requested the Out-of-State Criminal History Record during the inspection.
Plan of Correction: Karter Schools will construct each staff file prior to hire and have it approved by our state licensing rep to ensure all documentation is completed and
within accordance with licensing standards. A staff member will
not start until their entire staff file has been approved by VDOE.
