During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 2/04/2025 in response to a complaint received by the licensing office on 2/03/2025 relating to allegations of supervision and procedures of emergencies. The inspector reviewed one child's record and one staff record on-site on 2/04/2025. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to operational responsibilities and medications, and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/12/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The center's policy regarding medication states "Karter will administer over-the-counter
medications at the request of the parent/guardian in accordance with VDSS. The parent/guardian is responsible for supplying the over-the-counter medication as well as completing and submitting a VDSS 'Medication Authorization Form'."

The center did not follow this policy on January 14, 2025 when Medication #1 was administered to two year old Child #1 without receiving a request to do so from the
parent/guardian and without a completed medication authorization form from the parent/guardian. Medication #1 belonged to the center and was not supplied by the parent/guardian.
Plan of Correction: The center will retrain ALL staff on Karter's medication policy on
their next Professional Development Day.
Standard 8VAC20-780-510-B
Prescription and nonprescription medication shall be given to a child according to the center's written medication policies; and only with written authorization from the parent.

Though there was no written parent authorization to administer a nonprescription
medication to two-year old Child #1, Medication #1 was administered to Child #1 on January 14, 2025 at approximately 12:06pm. Medication #1 belonged to the center and was not supplied by the parent/guardian.
Plan of Correction: The center will retrain ALL staff on Karter's medication policy during
their next Professional Development Day.
