During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 9/08/2025 and completed on 9/10/2025 in response to a complaint received by the licensing office on 9/02/2025 relating to allegations of Staffing and Supervision and Programs. The inspector reviewed three staff records on-site on 9/08/2025. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, violations that were not reported were found related to meal times and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/18/2025. A POC submitted after this date will not appear on the public website.
Violations
3When staff are supervising children, they shall always ensure their care, protection, and guidance.
On August 25, 2025, while sitting at a table a 2 year old child pushed their chair against the table causing them to fall backwards and hit the floor. The child became visibly upset. Three staff were working in the classroom and did not provide care, protection or guidance to the child. All three staff stated they did not see the child fall.

On 8/25/2025, a two year old child fell backwards in a chair and began to cry. All three staff did not interact with the child after the fall and did not provide the child help, support or comfort. The child did get up off the floor by herself. The child was still crying when she walked to a staff member who was behind a counter. The staff member guided her back to the table stating "let's go sit down" and did not comfort the crying child.
On 8/25/2025, two (two year old) children were pulling at the same toy at the table. A staff member saw the incident and walked over and grabbed the toy from both children rather than promoting ways of getting along.
