Inspector Notes

An unannounced inspection was initiated on and completed on 9/16/2025 in response to a complaint received by the licensing office on 9/12/2025 relating to allegations of staffing and supervision..The Department issued a Notice of Intent to place the licensee on probation and require licensee to contact parents, guardians or other responsible persons regarding health and safety violations.on 9/11/2025 as a result of high-risk violations

The inspector reviewed three children?s records on 9/16/2025. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. The required ratio for children age 2 years old is one staff per eight children.

On September 15, 2025, the daily student attendance record documented the Tokyo classroom being over ratio from approximately 9:02am until 4:08pm (youngest child 35 month old) with two staff supervising.(20 children until 2:14pm, 18 children until 4:08pm).

On September 16, 2025, two staff were observed supervising 20 children in the Tokyo classroom (youngest child 35 month old) from approximately 10:50am until 1:00pm. The attendance record documented the classroom being over ratio from approximately 9:00am until 1:00pm. (20 children until 11:08am and then 19 children until 1:00pm).

is 35 months old had agreed and signed a ratio
modification waiver, along with the teachers and
leadership, which allows the child and classroom to
function at an appropriate 1:10 ratio

With a parent's written permission and a written assessment by the program director and program leader, a center may choose to assign a child to a different age group if such age group is more appropriate for the child's developmental level and the staff-to-children ratio and group size shall be for the established age group.

Child A (at age 34 months old) was moved to the Florence 3 year old Classroom on 8/25/2025 without the program directors written assessment.

Child B (at age 35 months old) was moved to the Tokyo 3 year old Classroom on 9/15/2025 without parent's written permission and a written assessment by the program director and program leader.
Plan of Correction: Corrected on the same day, the parents of the child who
is 35 months old had agreed and signed a ratio
modification waiver, along with the teachers and
leadership, which allows the child and classroom to
function at an appropriate 1:10 ratio. The other ratio modification
waiver for the 34 month old was signed by Leadership.
