Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 2/04/2026, as a part of probation and the provisional licensure period, and an Intensive Plan of Correction (IPOC).

The Department issued a Special Order to place the licensee on probation and require licensee to contact parents, guardians or other responsible persons regarding health and safety violations on 10/02/2025 as a result of high-risk violations. The Department requested that the center complete an IPOC as result of the 9/08/2025 and
9/16/2025 inspection due to the number, serious nature, and/or repetition of violations found. The Department requested that the center complete an IPOC as result of the 11/06/2025 inspection due to the number, serious nature, and/or repetition of violations found. The Department issued a Special Order to require licensee to contact parents, guardians or other responsible persons regarding health and safety violations on
1/12/2026 as a result of high-risk violations.

The on-site inspection began at 8:36am and ended at 10:32am. The inspector reviewed compliance in the areas listed above. There were 130 children present and 27staff. The inspector reviewed 5 children?s records and 5 staff records on-site on 2/04/2026. This inspection included document review, tour of the facility, interviews, and observations.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
