During the inspection, the inspector reviewed the areas listed. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 4/09/2026, as a part of probation and the provisional licensure period, and an Intensive Plan of Correction (IPOC).
The Department issued a Special Order to place the licensee on probation and require licensee to contact parents, guardians or other responsible persons regarding health and safety violations on 10/02/2025 as a result of high-risk violations. The Department requested that the center complete an IPOC as result of the 9/08/2025 and9/16/2025 inspection due to the number, serious nature, and/or repetition of violations found. The Department requested that the center complete an IPOC as result of the 11/06/2025 inspection due to the number, serious nature, and/or repetition of violations found. The Department issued a Special Order to require licensee to contact parents, guardians or other responsible persons regarding health and safety violations on 1/12/2026 as a result of high-risk violations.
The on-site inspection began at 9:17am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 154 children present and 40 staff. The inspector reviewed 10 children?s records and 7 staff records on-site on 4/09/2026. This inspection included document review, tour of the facility, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
