Inspector Notes

An unannounced subsidy health and safety inspection was conducted on 5/23/2024 beginning at 2:00pm and ending at 2:45pm. There were 2 children in attendance at the time of the inspection. Required background searches, training certificates and household member information was reviewed. A sample of requirements for daily attendance, child information/records, emergency preparedness planning and supplies were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the vendor. Based on review of children's information, it was determined that written authorization for emergency medical care and parent work phone numbers were not documented.

Evidence: There was no written authorization for emergency medical care for Child #1 and Child #2. There was no phone number for the parent of Child #1.
Plan of Correction: Vendor will obtain emergency medical authorization for both children and work phone numbers for the parent of Child #1.
