Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 5/23/2025. The inspection started at 10:35am and concluded at 11:40am. There were two children present and one caregiver; the point total was six. The inspector reviewed ## children?s records and ## caregiver/household members records on site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. The vendor had not recorded attendance in written hard copy format for the past seven months.

Standard 8VAC20-790-230-A
Before a child may attend the family day home, the vendor shall obtain documentation that the child has been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations. Child #4 has been attending for over two months without the vendor obtaining an immunization record for them.

Standard 8VAC20-790-230-B
The vendor shall obtain documentation of additional immunizations once every six months for children less than the age of two years. The vendor did not obtain documentation of additional immunizations for Child #2, who is missing records of their 18 month immunizations.

Standard 8VAC20-790-390-E
The diapering surface shall be separate from the kitchen, food preparation areas, or surfaces used for children's activities. The vendor was changing a child's diaper on a bench used for children's activities.
