An announced initial inspection was conducted on 05/18/2023. Inspector arrived at 11:30am and concluded the inspection at 12:30pm. There were no children present during the inspection.

Based on review, the center failed to ensure the emergency plan shall contain procedural components for lockdown, shelter-in-place and emergency evacuations.

Evidence: The center did not have an emergency evacuation plan specified for the site containing exit routes or plans for emergency situations or evacuations.

Based on observation, the center failed to provide a 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone.
