Inspector Notes

An unannounced monitoring inspection was initiated on 7/24/2024 and completed on 9/09/2024 in response to a self-report received by the licensing office on 7/05/2024 relating to an injury of a child requiring outside medical treatment. The inspector reviewed 1 child?s records and 5 staff records on site on 7/24/2024. This inspection included, document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/16/2024. A POC submitted after this date will not appear on the public website.

Centers shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. Child 1 received outside medical treatment for an injury sustained while in care at the center. It was noted on the injury report that the child was taken to the doctor, but the Department was not notified.
