Inspector Notes

An unmandated subsidy health and safety (SHSI) inspection was conducted on April 26, 2024 from approximately 11:30am-12:20pm. There were 5 children present with 2 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program,

Based on observation, the vendor exceeded the capacity of the children allowed by law.

Evidence: The vendor was caring for 5 children. The vendor has a permit to care for maximum of 4 children.
Plan of Correction: I will not take care of more than 4 children.
Standard 8VAC20-790-190-A
Based on record review, the vendor did not maintain a record for each enrolled child.

Evidence: A child receiving care did not have a file.
Plan of Correction: I will make sure to gave a file for the children.
Standard 8VAC20-790-190-B
Based on record review, the vendor did not ensure all children had a complete record.

Evidence: Child 2 and 3 did not have a proof of identification completed.
Plan of Correction: I will make sure to document when I see the original identification.
Standard 8VAC20-790-430-A-2
Based on record review, the shelter in place was not practiced twice a year.

Evidence: The vendor has not practice two shelter in place drills in this year.
Plan of Correction: I will make sure to do the drills.
