Inspector Notes

An unannounced non-mandated inspection, as required by the conditional license was conducted on 07/06/2023 and concluded with an electronic record review on 07/11/2023. There were 46 children, ages 5 years- 12 years, and 9 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies, nutrition and background checks. The children were observed mealtime, self-directed activities, and teacher-directed activities. A total of 5 children?s records, 5 staff records and 4 medications were reviewed. The inspector discussed the following with the staff: volunteers and medication. The inspector arrived for the inspection at 8:45 am and departed at 10:45 am.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to provider.

Standard 8VAC20-780-560-G
Based on observation and interviews, the center failed to ensure that when food is brought from home the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence:
1. There were approximately 4 lunch boxes in Group 1 that were not dated or labeled to identify the owner. The staff verified that the food was brought from home.
2. There were approximately 8 lunch boxes in Group 3 that were not dated or labeled to identify the owner. The staff verified that the food was brought from home.
Plan of Correction: Staff will label food from home with date and name of owner.
Standard 8VAC20-780-80-A
Based on observation and discussion with staff, the center failed to ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. The attendance record in Group 2 indicated 10 children present. There were 11 children in care at the time the attendance record was reviewed. The attendance did not document the arrival as it occurred.
2. As of approximately 9:00 am on 07/06/2023, the daily attendance for Group 4 had not been taken. There was a total of 17 children in care. The daily attendance record did not document the arrival of each child as it occurred.
3. As of approximately 9:05 am on 07/06/2023, the daily attendance for Group 3 had not been taken. There was a total of 12 children in care. The daily attendance record did not document the arrival of each child as it occurred.
Plan of Correction: Staff will take attendance at the time of arrival.
