Inspector Notes

An unannounced renewal inspection was conducted with administrative staff at the centers home office on 10/10/2023. There were no children in care during the time of inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, and emergencies. A total of 5 staff records and 5 agent/applicant records were reviewed. The inspector arrived for the inspection at 12:15 pm and departed at 12:45 pm.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and interview with staff, the center failed to ensure the injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.

Evidence: Injury prevention procedures was last updated January 2022. Staff 6 confirmed that was the most recent update.
