During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 11-25-2024 and completed on 11-26-2024. The on-site inspection began at 2:05pm and ended at 3:55pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 staff. The inspector reviewed 5 children?s records and 5 staff records on-site and electronically on 11-26-2024.. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 12-5-2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #4?s updated screening was due over by 8 months.
Plan of Correction: The Director will make sure staff results are current.
Standard 8VAC20-780-240-B
Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Staff #5 does not have documentation of orientation training and has been working for more than 3 years.
Plan of Correction: The employees orientation was located and currently placed in her records.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. This list shall be dated and kept confidential in each room or area where children are present. The allergy list was not dated and listed children who are no longer enrolled.
Plan of Correction: The Director will update and date the current children with allergies and restrictions.

When centers choose to provide meals or snacks a menu listing food to be served for meals and snacks during the current one-week. The meu posted was for June 2024.

Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The attendance sheet for the 8 children in care only listed 5 children and 1 child marked present was not in care.
Plan of Correction: The Director will update the names on the Brightwheel app and make sure staff is doing daily attendance.
