An unannounced monitoring inspection was conducted with staff on August 28, 2023 beginning at and ending at. There were two children, ages 5 years old, in the care of three staff members. The children were observed playing tabletop games with the staff. 5 children?s records were reviewed. Two staff records were reviewed and in compliance. The first aid kit and flashlight were observed and in compliance. There were two medications reviewed and in compliance.

Based on a review of documentation, the center did not obtain documentation that each child has received the required immunizations prior to the first day of attendance. Evidence: Child A's record was missing documentation of immunizations. The first day of attendance was 8/21/2023.
Plan of Correction: The staff will request the missing documentation from the parents and it will be placed in the record upon receipt.

Based on a observation, the center did not have all of the required nonmedical emergency supplies. Evidence: There was no battery-operated radio in the emergency supply kit.
Plan of Correction: The center director stated that a battery-operated radio would be purchased immediately.
