Inspector Notes

An unannounced renewal inspection was conducted on October 26, 2023, from 10:00am-10:50am. At the time of entrance, (2 ) children (6 ) points were in care with ( 1 ) caregiver present. The inspector reviewed 2 children?s records and 1 caregiver/household member record. Children were observed during snack time, toy play, diapering and handwashing. No medications were on site. Areas of non-compliance are identified on the violation notice.

Based on a review of children records, the provider has not obtained documentation of a child's physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1, date of first attendance 6/22/2023 does not have documentation of physical examination. The child was observed in attendance this date.
Plan of Correction: The provider will ask the parent for a physical examination to be completed and the physical document will be filed in the child's record.
