During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 11/20/2024 and completed on 11/20/2024. The on-site inspection began at 9:45 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was (6). The inspector reviewed (2) children?s records and (2) caregiver/household members records.
This inspection included document review, tour of the facility, interviews, observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
6A substitute provider was in care today and all this week. There was no record of the time of arrival and departure for each day this week.
There were no attendance records on the date of inspection or this week. There were 2 children in attendance this day. The substitute provider did not take written attendance on November 20, 2024.
The record for Child #2 was missing name, address and phone number of 2 persons to call in an emergency, name and policy number of child's medical insurance, proof of child's identity, and written authorization for emergency medical care, The child was present during this inspection and has been in care for 4 1/2 months.
The parents for child #2 were not provided in writing the following required information: policies for administering medications and whether or not there is liability insurance. The child has been in attendance for 4 1/2 months.
The provider has not documented in writing the annual review of the emergency plan. The annual review is overdue by 6 months.
There was no documentation of emergency evacuation drills.
