Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.


Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-790-250-B
Within the first 90 days of employment or service all caregivers shall complete Virginia Preservice Training for Child Care Staff sponsored by the Department of Education,
Caregiver #2 has not completed the Preservice training and has been caring for children for over 6 months.
Plan of Correction: The caregiver will take the Preservice 10 hour training.
Standard 8VAC20-790-250-D-1
All caregivers shall have within 90 days of employment or 90 days from
subsidy vendor approval: Current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Caregiver # 2 does not have current CPR, it has been expired for 4 months.
Plan of Correction: The caregiver will renew the CPR certification.

All caregivers shall have within 90 days of employment or 90 days from
subsidy vendor approval: Current certification in first aid appropriate to the ages of children in care.
Caregiver # 2 does not have current first aid, it has been expired for 4 months.
Plan of Correction: The caregiver will renew the first aid certification.

REPEAT VIOLATION
Caregivers who work directly with children shall
annually attend at least 16 hours of training, to include the department's health and safety
update course.
Caregiver #1 does not have documentation for taking the annual health and safety update
training which is past due by 1 month.
Plan of Correction: The Provider will take the 3 hour annual health and safety update course and file the certification in her record.
