During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated on 04/17/2025 and completed on 04/28/2025 with a local agency in response to a complaint received by the licensing office on 04/04/2025 relating to allegations of discipline. The inspector reviewed 1 children?s record and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Health and Safety Checklist and Background Checks and are documented on the violation notice issued to the program.

The provider is required to obtain a completed sworn statement prior to the employee's first day of employment.
Caregiver 2, who had been employed over 7 months, did not have a completed sworn statement.

No person shall be hired for compensated employment prior to requesting the results of the central registry search and the provisional hire shall not be alone with children until the results of the central registry search have been obtained as required by the Code of Virginia.

Caregiver 2, who had been employed over 7 months and had worked alone with children, did not have documentation of requesting a search of the central registry.

The provider shall meet the criteria on the health and safety checklist at the time of the initial evaluation or monitoring visit.
