Inspector Notes

An unannounced complaint inspection regarding supervision was conducted on May 2, 2024 from 12:30 p.m. to 1:30 p.m.

Staff interviews were conducted. Documentation was reviewed. Observation conducted.
Additional interviews were conducted off site.

An exit interview was conducted with the director on May 24, 2024.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it within five (5) calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s)

Based on staff interviews and review of documentation, staff failed to ensure that children under the 10 years of age were always within actual sight and sound supervision of staff.
Evidence:
1.On April 24, 2024 at approximately 10:45 a.m., Staff A left Infant A, age 12 months, alone sleeping in a crib in the infant room for approximately 10 minutes.

2.On a second occasion Staff A left the infant room, leaving infant A, age 12 months, and Infant B, age 8 months alone in the classroom. One infant was in a door frame jumper between the infant classroom and PreK classroom and the second infant was lying on a boppy pillow drinking a bottle by themself for approximately 10 minutes.

Based on staff interviews and review of documentation, the center failed to ensure that the required staff to children ratio of one staff with four infants was maintained at all times. Evidence: On April 24, 2024, there were two infants with no staff for approximately 10 minutes at least two different times.
