Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on July 30, 2024. The on-site inspection began at 11:00 a.m. and ended at 1:00 p.m. The inspector reviewed compliance in the areas listed above. There were 26 children present and four staff.
The inspector reviewed five staff records on-site.This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 19, 2024. A POC submitted after this date will not appear on the public website.
Violations
4Evidence:
1. In the infant room, there was a loose, unsecured, hanging extension cord behind the diapering trash can that created a risk of entanglement or injury by plugged in items falling.
2. In the two's room, a cordless vacuum was stored unsecured against a wall creating a fall risk.
Based on observation, the center failed to ensure children under the age of 10 years old were within actual sight and sound supervision of staff. Evidence: On July 30, 2024, Staff #2 was observed leaving the preschool classroom of ten children ages 3 years to 5 years old alone. This occurred multiple times lasting from one to two minutes each time.
