Inspector Notes
A monitoring inspection was conducted on 11/1/23 from 10:25am until 12:10pm. At the time of the inspection, there were 20 children in care with 8 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed participating in learning activities and arts and crafts projects, engaging in free play and eating lunch. Lunch service and handwashing procedures were also observed. First aid and emergency supplies, the emergency preparedness plan, medication, children's injury reports, documentation of emergency practice drills and required center postings were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations are listed on the violation notice issued to the center and were discussed with the center owner during the exit interview.
Violations
3Evidence:
1. Staff 1 has a documented date of hire of 10/4/23. The results of the national fingerprint search for staff 1 was not received by the center until 10/6/23.
2. The center owner confirmed that staff 1 began employment prior to the results of the national fingerprint search being received.
Evidence:
1. Staff 2 (date of hire: 9/5/23) indicated on her sworn statement or affirmation that she has resided in the state of new York within the past five years. The record for staff 1 does not contain the results of a search of the child abuse and neglect registry from the state of New York.
2. The center owner confirmed that a search of the child abuse and neglect registry from the state of New York was not obtained for staff 2.
Evidence:
1. The records for child 1, child 2, child 3, child 4 and child 5 did not contain the addresses of the two designated people to call in an emergency if a parent cannot be reached.
2. The records for child 2 and child 4 did not contain the work phone number for the parent.
3. The center owner confirmed that the records for the above listed children do not contain all the required information.
