The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry or a search of the sex offender registry from one state for staff 1, who has been employed for 7 months. The center did not request a search of the child abuse and neglect registry or a search of the sex offender registry from one state for staff 2, who has been employed for 2 weeks. The center did not request a criminal history check, a child abuse and neglect registry check or a sex offender registry check from one state for staff 3, who has been employed for less than one week.
Plan of Correction: The center responded with the following: The out-of-state background checks for staff 1, staff 2 and staff 3 will be completed and results will be placed in each staff's record once recieved. Going foward, center management will ensure that all required background checks are completed within the required timeframes.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. There was a spray bottle of sanitizer on an open shelf in the boys' bathroom.
Plan of Correction: The center responded with the following: The sanitizer was removed from the open shelf in the bathroom at the time of the inspection. Center management will ensure that all hazardous substances are kept in a locked place when not in use.
