Inspector Notes
An unannounced 60-day inspection was initiated and completed on 8/9/2023. The inspector was on site from 10:30am until 1:15pm.
There were 34 children, ages 2 months to 5years old under the supervision of 10 staff. Children were observed during outdoor free play, lunchtime and nap time. Four children's records, one medical record and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations
4Evidence: Staff #4 was working day of inspection on 8/9/2023. Staff #1 had a TB screening dated 08/10/2020.
Evidence:
There was not a staff member on duty that had obtained instruction in performing the daily health observation of children during the 08/10/2023 inspection.
Evidence:
1.In the Early Pre-K room there were hazardous chemicals unlocked over the sink.
2.In the adjoining infant/ toddler bathroom there was a Swiffer with the chemicals attached to the cleaning device. The doors to both rooms were unlocked.
Evidence: In the infant room fridge, Inspector observed two infant bottle containing prepared formula. Bottles were not labeled with a child?s name or date.
