Inspector Notes
An unannounced renewal inspection was initiated and completed on 12/8/2023. The inspector was on site from 10:00am until 1:20pm.
There were 30 children, ages 4 months to 5years old under the supervision of seven staff. Children were observed during indoor free play and lunchtime. Five children's records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

8Evidence: Staff #4, hire date of 7/31/2023 had an fingerprint eligibility letter on record dating 10/10/2023 during the inspection on 12/8/2023.
Based on record review, the center failed to ensure each staff member has a documented TB test or screening submitted at the time of employment and prior to coming into contact with children.
Evidence: Staff #1 has a hire date of 7/3/2023 had a tb screening dated 9/14/2023
Staff #2 has a hire date of 5/22/2023, had a tb screening dated 9/20/2023.
Staff #3 has a hire date of 7/3/2023, had a tb screening dated 10/18/2023.
Staff #4, has a hire date of 7/31/2023, had a tb screening dated 11/29/2023.
Staff #5 has a hire date of 7/3/2023, had a tb screening dated 1/28/2023.
Evidence: In the early Pre-K class, the diapering pale would not close granting access to children.
Evidence: In the toddler class children?s lunchboxes were hanging on hooks under cubbies. Some food had been put in the children?s cubbies, while other food was left in the lunchboxes. These lunchboxes were not labeled with name or dates.
Evidence: In the toddler classroom a child was observed walking around drinking a bottle.
Evidence:
In the infant classroom, there was an unlabeled container of opened baby food in the refrigerator. Staff #6 stated that a child had been fed out of the container of food earlier that day.
Evidence:
Staff #4has a hire date of 7/31/2023, had no record of a current Certification in cardiopulmonary Resuscitation during the inspection on 12/08/2023.
Evidence:
Staff #4 who has a hire date of 7/31/2023, had no record of a current certification in first aid during the inspection on 12/8/2023.
