During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 2/18/25. The on-site inspection began at 12:05pm and ended at 1:40pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and 5 staff. The inspector reviewed 5 children's records and 4 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There is no physical examination in child 1's record, who has been enrolled for four months.
Plan of Correction: The center responded with the following: The parent of child 1 was contacted during the inspection and a copy of the most recent physical was requested. Once received, it will be placed in the child's record.

In areas used by preschool age or younger children, electrical outlets shall have protective covers.

In the infant room, there was an electrical outlet without a protective cover next to an occupied crib.
Plan of Correction: The center responded with the following: This was corrected during the inspection. All staff will be reminded that electrical outlets must have protective covers when not in use.
Standard 8VAC20-780-450-A
Cots used by children during the designated rest period shall have linens consisting of a top and bottom cover.

There was a cot being used by a child during nap time that did not have a top cover.
Plan of Correction: The center responded with the following: Extra linens are kept at the center. Staff will be reminded to provide children with nap linens if they do not have their own. Parents will also be reminded to bring linens back to the center after being taken home to wash.

Staff records shall include a name, address and telephone number of a person to be notified in an emergency.

The record for staff 1 does not include emergency contact information.
Plan of Correction: The center responded with the following: Staff 1 filled out the emergency contact form during the inspection and it was placed in the staff record.
