Standard 22.1-289.031-A-3-c
The required ratio for children ages 16 months to 24 months is one staff per five children.

Staff 2 was the only staff member supervising six children ages 22 months- 4 years; Child 3 (age 22 months) was identified as the youngest child in the group. Two staff were required for this group of children.
Plan of Correction: The child was moved back into the infant class to ensure ratio is met.

Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.

There was not a record of a health exam on file for Staff 3 or Staff 4. The health exams for Staff 2 and Staff 5 were more than a year overdue.
Plan of Correction: Will ensure all health exams are done before employment and annually.

The center must request a search of the central registry prior to the employee?s first day of employment.

A central registry search for Staff 1 was dated more than 5 months after the employees? first day of employment. There was not a central registry search for Staff 3 available for review; Staff 3 was working alone with children.

For each group of children, the vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

The vendor did not maintain a written hard copy record for each group of children.

Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB); documentation of the screening shall be submitted at the time of employment, prior to coming into contact with children, and shall be been completed within the last 30 days.

There was not a TB screening available for Staff 1, Staff 3, or Staff 4.
Plan of Correction: The staff will get a screening.

All staff who work directly with children shall have within 90 days of the date of employment a current certification in CPR; the training shall include an in-person competency demonstration.

The CPR certification for Staff 2, who has been employed for 22 months, has been expired for 6 months. Staff 3, who has been employed for 7 months, does not have a current certification in CPR.

The diapering area shall have a sink with running warm water and an appropriate disposal container required by the standards.

The diapering area in Classroom 1 did not have a sink. The disposal container was positioned in a way that did not allow for use in a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the system.
Plan of Correction: The diaper changing area was moved out of the class.
