Standard 22.1-289.031-A-3-c
The required ratio for children ages 24 months to 36 months is one staff member per 8 children.

Staff 1 was the only staff member supervising 12 children, ages 2 years-4 years, from approximately 7:45 am to 8:30 am on 02/11/2026; 2 staff were required for this group of children.
Plan of Correction: I have hired a full time employee starting in two weeks.

Children less than 10 years of age shall always be within actual sight and sound supervision of staff.

Staff 2 was in the hallway of the center leaving the 7 children (ages 2-4 years) in Class 2 out of sight and sound supervision of a staff member on 02/19/2026. Staff have to leave classrooms to get items out of the children?s cubbies located in the hallway, resulting in the children being out of the sight and sound supervision of staff.
Plan of Correction: I moved the toddler rooms closer together/moved the entrance to the infant room.

The following action or threat thereof is forbidden: enclosure in any space that the child cannot freely exit himself; this does not apply to a highchair when used for its intended purpose.

Staff 2 placed Child 1 (age 2 years) into a portable highchair attached to a chair for time-out (for two minutes) on two different occasions. The child could not freely exit the highchair which was not used for its intended purpose.
Plan of Correction: We have dismantled the chair and made the church aware not to use it on Sundays as well.
