Inspector Notes

An unannounced monitoring inspection was initiated and concluded on 01/24/2024. At the time of entrance, 11 children totaling 24 points were in care with the provider and assistant provider. The record sample size consisted of 5 children's records and the provider's record and the assistant's record. Medications were also reviewed. The children in care were observed engaging with the provider and assistant and lunchtime. There have been no injuries since the last inspection. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

Based on record review and interview it was determined the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. The record for child #4 (enrolled 9/5/23) did not contain documentation of a physical exam.
2. provider confirmed there was no documentation of a physical exam in the record for child #4.
Plan of Correction: Provider will request documentation of a physical exam from parent.
