During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced monitoring inspection was initiated and completed on 11/18/2024 in response to a self-report received by the licensing office on 11/15/2024 relating to Administration and Supervision of children. This inspection included document review, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection and guidance.
1. Staff # 1 and Staff #2 lined their 2 classes up on the sidewalk of the playground to do a head count to come inside at approximately 4:53 pm. Staff #2 was standing at the back of the lines and went to retrieve an emergency backpack left on the playground. When Staff #2 returned the lines were gone and assumed that Staff #1 had taken both classes of children to the afterschool classroom.
2. Staff #3 and Staff #4 in an adjacent classroom to the playground observed the child alone on the playground outside their window. Staff #3 opened the outside door and brought the child inside at approximately 4:56 pm.
Plan of Correction: Staff must complete the Name-to-Face (NTF) procedures to ensure all children are accounted for before leaving the playground and after entering the building. Staff should stop at the checkpoints to ensure their counts as well.
Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
At approximately 4:54 pm on 11/15/2024 staff #1 and #2 lined up to bring the children inside from the playground. Child #1 ( age 3 years) was left alone on the playground for approximately 2 minutes or less. Staff #3 and Staff #4 in an adjacent classroom observed the child alone on the playground outside their window, Staff #3 opened the outside door and brought the child inside. Child #2 was brought to administration at 4:56 pm.
Plan of Correction: One staff member must walk the perimeter of the playground after all students have entered the building.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center?s activities and services are maintained in compliance with the center?s own policies.
Staff #1 failed to follow the center?s own policy in regards to leaving the playground, resulting in a child being left alone.
The staff handbook states ?Every employee must know the number of children they are supervising at each moment of the day?Complete a name -to-face Roster Destination & Time Spot Check and Verification/ Spot Check section when children are traveling outside of the classroom. ?
Plan of Correction: All spot checks and transfers will be recorded on the ProCare app. One teacher will have the written form of attendance and another teacher will use the tablet to document the movement of the children.
