During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 2/19/2025, as a part of the licensure period. The on-site inspection began at 10:00 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 85 children present and 17 staff. The inspector reviewed 3 children?s records and 3 staff records on-site
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Violations
2The gate to the fence on the outside playground has a jagged piece of wood on the top of the middle panel.
The floor in the gym has multiple planks that are separated and becoming disconnected from the base.
Two bottles of infant formula were not labeled with the date.
