Inspector Notes

An unannounced renewal visit and facility tour was initiated and concluded on 02/07/2024. At the time of inspectors' arrival there were 13 school aged children in care with 2 staff members. Children were observed interacting with staff and departing. A sample of 5 children's records and 2 staff records were reviewed. Staff reported that there have been no injuries and medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review and interview it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. The record for child #3, present during inspection, did not contain documentation of immunizations.
2. Staff #1 confirmed that the record for child #3 did not contain immunization documentation.
Plan of Correction: Staff will request immunization record from parent.
