Inspector Notes
An unannounced renewal inspection was conducted on February 22, 2024, at 4:50pm to approximately 6pm. There were 14 children present and one staff member. There were initially 19 children present with one staff at the opening of the center. Inspectors were able to review eight records including six children?s records and two staff members records. Children and staff interactions were observed during the entire inspection. There were no medications and transportation is not provided. The children were observed during free play and parent pick up. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site director at the exit interview.

1. In speaking with all 14 children as a group, they confirmed that there was only one staff member present at the after school program CDC.
2. Staff #1 confirmed that they were the only staff member present at the center supervising all 19 children in care.
3. Staff #3, program manager, confirmed that they were aware that there was only one staff member at the center to supervise children under the age of 12 years old.

Evidence:
1. Two children under the age of 10 years old were observed going to the water fountain and restroom without adult staff supervision located outside of the room.
2. Staff #1 was observed prompting other children in care to check on the children at the water fountain and restroom located outside of the room.

Evidence:
1. The youngest child at the center was 5 years old which the staff-to-children ratio is 1 staff member to 18 children. Present at the center were 19 children and 1 staff member.
2. Staff #1 confirmed that the center was out of ratio. The youngest child present was 5 years old and Staff #1 confirmed 19 children were present in care. There was no assistant teacher or additional staff at the center.
3. In reviewing the face to name attendance sheet, there were 19 children listed and one staff member starting at 3:10pm.
