Inspector Notes

An unannounced inspection was initiated on June 10 2025 and completed on June 17 2025 in response to a complaint received by the licensing office on June 9 2025 relating to allegations of Nutrition and Food Services. The on-site inspection began at 12:15pm and ended at 1:15pm. The inspector reviewed 18 children?s records and 0 staff records. This inspection included a document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on July 10 2025. A POC submitted after this date will not appear on the public website.

Children three years of age or younger may not be offered foods that are considered to be potential choking hazards.

During a classroom event on February 14 2025, Child #2, age 2 years 11 months, and Child #5, age 2 years and 11 months, were served whole grapes. According to the American Academy of Pediatrics whole grapes are considered to be potential choking hazards.
Plan of Correction: All staff have been re-trained on food safety guidelines. Grapes and other choking hazards will be cut appropriately (grapes will be quartered) for all children 3 years or younger. Teachers will verify all food is age-appropriate, especially for children who transition before their third birthday. Reminders are posted in prep areas, and protocols will be reviewed regularly.
