During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on October 16 2025 and completed on October 30 2025, in response to a self report relating to supervision. The on-site inspection began at 10:30am and ended at 3:00pm. The inspector reviewed compliance in the areas
listed above. There were 163 children present and 39 staff. The inspector reviewed 1 child record and 2 staff records. This inspection included a document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on November 18 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On Oct 9 2025, Child #1(age 23 months) was left in a classroom unsupervised for approx. 6-8 minutes. Staff #1 and Staff #2 were unaware that Child #1 was missing..
Plan of Correction: Retraining on this standard/policy has been provided to the entire faculty. Additionally, the administrative team conducts regular number checks during transitions to ensure continued compliance and accuracy during transitions.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On October 9 2025, Child #1 (age 23 months) was not within sight and sound supervision for approximately 6 to 8 minutes. Child #1 was located by their parent in the classroom's bathroom. Staff #1 and Staff #2 were not aware that Child #1 was missing from the class.
Plan of Correction: Retraining on this standard/policy has been provided to the entire faculty. Sign-in sheets are maintained to document participation. Additionally, the administrative team conducts regular number checks during transitions to ensure continued compliance and accuracy during transitions.
Standard 8VAC20-780-40-E
The operational responsibilities of activities, services, and facilities are maintained in compliance with the center's own policies and procedures.

The center's policy states that ?As children leave the classroom with their teachers, threshold counts and name-to-face checks are conducted to ensure that all children are accounted for with the group.? The policy also states "Once the class has reached the final destination, a name-to-face check occurs. The teacher matches the name on the attendance record with the face of the child as he/she crosses the final threshold."

At approximately 4:30pm On October 9 2025, Staff #1 did not complete a name to face check at the threshold entrance of the classroom and at the threshold entrance of the playground.

Child #1 (age 23 months) was left in the classroom while Staff #1 and Staff #2 took the classroom outside. Child #1 was found by their parent in the restroom within the classroom, on a stool, at the sink with the water running approximate 6 to 8 minutes after the classroom entered the playground.
Plan of Correction: Retraining provided to all faculty on this policy. All staff observed performing correct name-to-face; each classroom cleared for compliance. Sign-in sheets documented, and admin conducts random number checks during transitions.
