Inspector Notes

An unannounced, on-site renewal inspection was initiated on March 20 2026 and completed on March 20 2026. The on-site inspection began at 9:30am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 173 children present and 27 staff. The inspector reviewed 10 children?s records and 10 staff records. This inspection included a document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 3 2026. A POC submitted after this date will not appear on the public website.

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

The center did not updated additional immunizations records for Child # 6 every 6 months. The last dated immunization record is dated 7/16/2025. The center is 2 months out of compliance.
