During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 11/5/25. The on-site inspection began at 11:34am and ended at 1:12pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 staff. The inspector reviewed 3 children?s records and 5 staff records while on-site. This inspection included document review, tour of the facility and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/13/25. A POC submitted after this date will not appear on the public website.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.

The wood mulch was less than 6" in depth. Beyond the top 1/2" layer of wood mulch the surfacing was compacted dirt.
Plan of Correction: In response to the identified non-compliance, the employees have contacted Fairfax County Park Authority Park Operations division to request the wood mulch be replaced to meet the 6" depth standard. The work is schedule to begin on Monday, November 17th and will take approximately 2 weeks. In the meantime, the playground is off limits for the preschool. Fairfax County Park Authority Park Operation employees will perform high frequency playground inspections to ensure compaction under equipment meets the critical fall heigh requirements.
