Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on 6/4/2025 and completed on 6/4/2025. The inspection started at 9:43 am and concluded at 10:35 am. There was 1 child present and 1 caregiver; the point total was 2. The inspector reviewed 5 children?s records and 2 caregiver/household member(s) records on-site and electronically on 6/3/2025.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/10/2025.
Violations
5At the time of the inspection, it was observed that Child #4?s arrival time was recorded on the attendance sheet as 6:34 am, however, the Vendor stated the child didn?t arrive until 9:43 am.
In the bathroom, it was observed that two disposable shaving razors were located in the tub, accessible to children.
In the bathroom, it was observed that a bottle of Listerine and an air freshener were sitting in the tub, accessible to children.
At the time of the inspection, it was observed that the Vendor did not wash Child #4?s hands after diaper changing.
At the time of the inspection, it was observed that the vendor changed Child #4?s diaper on the Vendor?s bedroom bed. The diapering surface used was absorbent and was not cleaned and sanitized after changing Child #4's diaper.
