Inspector Notes

An announced subsidy health and safety inspection (SHSI) was initiated and concluded on August 7, 2023. The licensing inspector was onsite from approximately 10:00am until 11:35am. There were four children in care one provider present. A monitoring inspection was also conducted on this date.

The information gathered during the inspection determined non-compliances with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard 8VAC20-790-190-B
Based on the review of four children?s records, the vendor did not maintain the required information in one child?s record.
1. The record for Child #2 (first date of attendance 07/10/2023) did not include the address of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care.
Plan of Correction: I will have the parent add the address for the listed emergency contacts.
Standard 8VAC20-790-230-A
Based on observation, the vendor did not obtain documentation that the child has been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations before two children began attending the family day home.
Evidence:
1. The record for Child #2 (first date of attendance 07/10/2023) contained immunization documentation dated 07/27/2023.
2. The record for Child #4 (first date of attendance 07/10/2023) contained immunization documentation dated 07/24/2023.
Plan of Correction: Going forward, I will not allow children to begin attending the family day home until all of the paperwork has been obtained.
