Inspector Notes
An unannounced renewal inspection was conducted on February 20, 2024, with the center director. There were 42 children present with 4 staff. The inspection began at approximately 3:15 PM and concluded at 4:30 PM. Children were observed playing on the playground, playing with age-appropriate toys and materials, and interacting with staff. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies, and nutrition. Transportation is not provided. A total of 5 children?s records, and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. The records of child #1 and child #2 did not contain immunization records.
2. Administration acknowledged that the immunization records were not received prior to attendance.
Evidence: The record for child #1 and child #2, did not contain a physical.
Evidence: Documentation was not maintained for emergency evacuation drills for the months of December 2023, and January 2024.
Evidence:
1. The record for child #1 (enrollment date 8/11/2023), did not contain parents place of employment and the parents work number.
2. The record for child #2 (enrollment date 8/16/2023), did not contain parents place of employment, the parents work number, and the address for 2 emergency contacts.
3. the record for child #4 (enrollment date 8/23/2023), did not contain parent?s work number, and the child?s doctor?s name, and phone number.
4. The record for child #5 (enrollment date 8/16/2023), did not contain the child?s doctors name, address and contact number.
