Inspector Notes
An unannounced renewal inspection was conducted on-site January 23, 2024. The director was available during the inspection. There were 26 children present, ranging in ages from 2 years to 4 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
4Evidence: In the Under the Sea (2yr-3yr olds) classroom, the thermostat reading was 62?F.
Evidence: 1. The Under the Sea classroom (2yr-3yr), had an unlocked cabinet under the sink in the bathroom containing cleaning materials, such as Lysol, bleach water, disinfectant spray, and cleaning duster. The labels on these products state "keep out of the reach of children" and "caution." 2.) Staff #3 confirmed the cabinet was unlocked.
3.) There was an unlocked closet in the same bathroom containing paint and Raid insect killer. The labels on these products state "keep out of reach of children" and "caution." 4.) Staff #3 reported there's never been a lock on the closet.
Evidence: 1.) In the library area there were 2 uncovered outlets. 2.) In the hallway, leading to the Under the Sea class, there was 1 uncovered outlet.
Evidence: 1. The records of staff #1 and staff #2 did not contain documentation of addresses for the designated emergency contact person.
2. The record of staff #2 contained documentation of two references that were not signed.
3. The record of staff #2, hired 10/12/23, did not contain documentation of orientation training. Staff #5 confirmed the training was completed but not documented.
