Inspector Notes
An unannounced monitoring inspection was conducted on 10/26/2023 between the hours of approximately 9:25 am and 10:30 am, to monitor the program?s compliance with the Standards during their first 60 days of operation under the 6 month Conditional Licensure period.
There were 4 children present, ranging in ages from 6 months to 2 years, with the provider supervising. At the time of entrance, children were eating breakfast. Breakfast today consisted of pancakes, apple slices, raisins and milk. After breakfast, children went to play with puzzles. Diapering was observed.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. No medication was on site today, according to the provider. The care area was observed and required posted documents were verified. Previous violations had been corrected. A total of 3 child records, 2 staff and 2 household members records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 8Evidence:
1) The record for Child #1 (start date 08/21/2022) did not contain documentation of a physical examination.
Evidence:
1) An electric outlet was observed not in use and without a protective cover.
Evidence:
1) At the time of entrance, there were four children in care with the provider. The record of attendance did not have children recorded in attendance for the day.
Evidence:
1) The record for Child #3 did not contain the Provisions of the Emergency Plan.
Evidence:
1) The most recent immunizations in the record for Child #1 (dob 09/20/2021) were dated 10/06/2022. Updated immunizations are due for Child #1.
Evidence:
1) The menu was not dated as required. 2) The menu did not list substitutions as required. Today's breakfast consisted of pancakes, apple slices and milk. The menu had listed bananas as the fruit to be served. The substitution was not listed.
Evidence:
1) On the date of inspection, the provider placed an infant (6 months old) in a crib and propped a milk bottle with blankets for him to drink.
Evidence:
1) Four children were seated on high chairs without the safety waist and crotch straps being securely fastened.
