Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 09/12/2024,as a part of the licensure period. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 17.
The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included * document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 09/26/2024. A POC submitted after this date will not appear on the public website.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises.
The findings from the most recent inspection, dated 01/24/2024, were not posted in a conspicuous place on the premises.

The family day home is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Caregiver #2, who had been employed for seven months, did not have documentation of requesting a central registry check from one state.

Standard 8VAC20-800-250-E
Balloons shall be inaccessible to children under five years of age.

One balloon was observed in an area accessible to children. All five children present were under five years of age.
Plan of Correction: Balloon was removed from daycare.

Children's records are required to contain the telephone number of each custodial parent's place of employment.

The record for Child #1 was missing the employment telephone number for one parent. The child was present during the inspection and had been in care for six months.

Before the child's first day of attendance, parents shall be provided required information in writing.

The parents of Children #1, #2, and #3 were not provided in writing the following required information: provisions of the family day home?s emergency preparedness and response plan. Child #2's parents were not provided with the policies for the administration of medications. Child #1 had been in care for two months, Child #2 had been in care for four months and Child #3 had been in care for seven months.
Plan of Correction: The parents signed the papers.

The provider shall review the emergency plan at least annually and update the plan as needed.

An update of the emergency plan was overdue by twelve days.

When food is brought from home the food container shall be clearly labeled in a way that identifies the owner.

Two milk bottles for Child #1 were not labeled with their name.
